JOB SUMMARY
The Case Manager, as part of the clinical team, can oversee and/or conduct comprehensive intakes and assessments of young adults/residents. The incumbent uses available data to create an individualized needs and services plan (NSP) that identifies the youth’s strengths and weaknesses to plan short and long-term goals. The Case Manager also cultivates a working knowledge and relationship with relevant community resources to meet the essential needs of young adults. Works closely with community behavioral health providers, and internal service teams to ensure coordinated and effective services. Provides an intensive array of services and supports, including life skills, basic needs assistance, such as Medi-Cal, and referral to community resources and mental health services. Provides crisis prevention, intervention, and safety planning services, along with stress management and life development, while adhering to laws pertaining to confidentiality, reporting requirements, and maintaining necessary compliance documentation.

CASE MANAGER
- Consistently contributes to the Organization’s Vision and Mission Statements and demonstrates the Values of the organization.
- Attends IEP, TBS, and Court (if needed).
- Assists Facility Managers with School Enrollment.
- Follow up with 1-on-1 meetings with residents to check-in with their case plan and ask ifthey have any issues or concerns at the moment with staff or residents, in addition to anyneeds.
- Attends child and family team (CFT) meetings.
- Coordinates or assists, scheduling, and ensuring all 30-day appointment (medical, dental, vision) visits for all residents within thirty (30) days of placement and the completion of proper documentation.
- Coordinates attendance, attends and represents SMGC at resident monthly school, probation, social work, and reports on results to the team.
- Connects the residents to services they are required per SW/PO.
- Works along with the Facility Manager to oversee all aspects of the facility to ensure the safety of residents and staff.
- Maintains all residents, facility, and staff records.
- Ensures compliance with all aspects of Title 22.
- Provide Case Management.
- Handle Crisis Intervention.
- Provide Supportive Sessions to Residents on assigned caseload.
- Gains resident specific knowledge of the DSM, special education, medication dosages, symptoms, side effects, and Juvenile Codes.
- Expedites admission for placement of children in Agency certified homes. Will transport children, as required.
- Performs written needs assessments, develops client’s treatment plans and their updates, if needed.
- Collaborates in the development of treatment plans with other program treatment consultants.
- Meets weekly with other Facility Social Workers to discuss plans for the residents.
- Attends court on behalf of clients as needed.
- Coordinating the emotional and social needs of the resident with the staff.
- Planning and attending all referral appointments (i.e. medical doctor, psychiatrist, court appointments, school appointments, etc.)
- Meets with facility managers, residents, and resident’s parents/relatives or significant other, as required; supervises and/or arranges visits between children and their parents, relatives; maintains documentation of such visits.
- Assists with writing, sending treatment/quarterly reports to placing agencies on time; responsible for proper documentation of client case records, treatment plans, treatment evaluations, termination, and discharge reports; participates in treatment planning sessions.
- Assists with documentation of foster home certification records and maintenance of ongoing certification and program standards.
- Assists with documentation, implementation and follow up of medical, dental needs for children and for ensuring the food and clothing needs of children are being met.
- Assists with reporting any actual or suspected child abuse; responsible for reporting special incident reports (both telephonic and written) to appropriate persons and/or licensing and placing agencies.
- Assists with documenting any violations and deficiencies of Agency certification standards, program standards, or state law as they pertain to foster parents or foster homes and for reporting these violations and/or deficiencies to the appropriate agencies and the administration for correction; formulates and implements plans of correction to correct deficiencies.
- Conducts or participates in investigations involving complaints, allegations, or matters pertaining to deficiencies and non-compliance with regulations, applicable laws, and Agency policies and procedures; provides input about re-certification and appeal proceedings.
- Must be available and accessible to work a flexible schedule as needed, including evenings and weekends.
- Obtain and maintain the appropriate business/professional licenses and/or certifications as required by Stella Maris Growth Center (SMGC) and provide evidence of such to his/her supervisor.
- Ensures open communication and problem resolution with resident’s schools or academic programs.
- Enrolls resident in school no later than two (2) days after receiving all required documents.
- Attends all special education meetings and student study team meetings to advocate for youth.
- Attends all IEP/504 meetings for each assigned resident assigned on their caseload.
- Provides documentation of all clients’ meetings within 24 hours.
- Provides clients and Facility Administrator with Resident Update form for all meetings even when meetings occur outside normally scheduled hours.
- Notifies youth and Facility Administrator of Incident Reports, furloughs, AWOLs, school suspension or disruptions, complaints, illegal activities, or requested contact with outside people.
- Requests approval from clients for residents to be relocated.
- Maintains a professional relationship and communicates with residents, clients, and staff ina respectful manner.
- Ensures the accuracy of paperwork submitted to the Facility Administrator.
- Provides clear, timely, and accurate 40 hours of training within one (1) year of hire date.
- Files residents’ and clients’ paperwork in designated binders
- Responds to voicemails and inquiries within for (4) hours of receipt or by the end of shift.
- Elevates issues promptly and to the appropriate person.
- Coordinates with corporate staff for the use of the corporate vehicle within two (2) business days, barring emergencies.
- Works with the Facility Administrator to ensure staff paperwork is accurate and maintained in compliance with procedures and protocols.
- Be part of the Emergency Response team for one on one and/or 5150 calls.
- Give full attention to what social service clients are saying, understand the points being made, ask questions, and not interrupt inappropriately.

MINIMUM QUALIFICATIONS:
- The Case Manager shall be an individual who has a Bachelor’s degree and two years of experience in a mental health setting as a specialist in the fields of physical restoration, social adjustment, or vocational adjustment.
- Up to one year of graduate professional education may be substituted for the experience requirement on a year-for-year basis.
- Up to one year of post-associate’s degree clinical experience may be substituted for the required educational experience in addition to the requirement of two years’ experience in a mental health setting.
- Furthermore, the case manager will work in conjunction with the activities coordinator for the planning and coordination of activities for our SMGC facilities. The designated staff member shall be assisted when needed to support all minor dependents in accordance with their needs, interests, and abilities to ensure inclusive participation. Responsibilities include the development, organization, implementation, and evaluation of the SMGC activity program as a whole and will require the assigned individual to complete or enroll in a related educational or training program and have six months of experience in constructing and arranging planned activities.As a condition of continued employment, must complete and accrue 40 hours of in-service training within one (1) year of hire date.CERTIFICATES, LICENSES, REGISTRATIONS:Incumbents are required to meet standards set by the State of California Community Care Licensing: physical exam and health questionnaire; Tuberculosis screening clearance; Statement of Criminal Convictions; Child Abuse Index Check; and Department of Justice Fingerprint Clearance. Required to possess and maintain a current and valid California driver’s license, acceptable DMV record and maintain the California minimum vehicle comp and collision insurance.
