The Case Manager, as part of the clinical team, can oversee and/or conduct comprehensive intakes and assessments of young adults/residents. The incumbent uses available data to create an individualized needs and services plan (NSP) that identifies the youth’s strengths and weaknesses to plan short and long-term goals. The Case Manager also cultivates a working knowledge and relationship with relevant community resources to meet the essential needs of young adults.
The Clinician provides mental health support services to members served and their families within the program. The Clinician also assists in the coordination of all mental health support services provided by the Team. This includes coordinating and collaborating with team members and outside agencies such as the Probation Department and County Behavioral Health Services.
The Direct Care Staff (DCS) will provide general supervision of residents in accordance with the policies of the Stella Maris Growth Center (SMGC) facility. DCS will be tasked with maintaining basic disciplinary and behavioral control of the residents while in the home. DCS will supervise residents during agency approved activities and will provide trauma informed care and supervision.
